What if I outlive my six-month prognosis?
Patients may “outlive” past their six-month prognosis, often due to the improved quality of life brought upon by hospice care.
After six months, our doctor will evaluate if the patient is still eligible and can benefit from hospice care. In this case, we can continue providing the service. If the patient is no longer qualified for hospice care due to an improvement in their condition, then they will have to be discharged. A referral to a home health agency can also be made if appropriate. Patients can also be readmitted at a later date should their condition worsen.
Hospice care includes:
- Visiting Staff (doctors, nurses, social workers, chaplain, and volunteers)
- Medical Supplies and Equipment
- Continuous Care (Crisis Care)
- Respite Care for Primary Caregivers
- Short-Term In-Patient Care
- Grief and Bereavement Services for Family Members and Caregivers (Up to one year after the patient’s passing)
